Amy Amitay, LMHC
Psychotherapist . Sex Therapist . Addiction Specialist
Do you take insurance?
Although I do not take health insurance, I am an out-of-network provider. Most out-of-network plans will cover anywhere from 40-100% of the cost of the session fee.
How does the process of out-of-network benefits work?
If your insurance company has out-of-network benefits, you pay me directly for each session. I then provide you with a receipt [Superbill] after each session, which you submit to your insurance company for reimbursement. Insurance claims, such as these, can typically be completed online with your insurance company.
How do I know if I have out-of-network benefits?
You want to ask the following questions of your insurance provider:
-
Do I have out-of-network mental/behavioral health benefits?
-
Is there coverage for couples/family sessions? [if applicable]
-
Do I have an out-of network deductible? How much is it? Has it been met yet by other medical expenses?
-
What amount/percentage of the psychotherapy fee is covered?
-
Is there a limit to how many sessions are covered per year? If so, what is it?
Do you offer sliding scale?
Yes. I set aside a certain number of appointments for sliding scale clients.
What methods of payment do you accept?
I accept credit card.
What is your cancellation policy?
If cancellation occurs in advance of 24 hours before the session time, there is no charge. In this case, I will make every effort possible to reschedule. However, if the cancellation occurs less than 24 hours before the session time, you are responsible for payment of the full session fee.
What is your lateness policy?
A scheduled appointment represents a commitment from both the therapist and client. More than half an hour late constitutes as a late cancellation, meaning you are responsible for the full session fee. I may work with you for the remaining time. However, this is up to my discretion and you will still be charged the full session fee.
Couples/Family Therapy - sessions will not begin if all participants aren't present.
How often will we meet for psychotherapy sessions?
Sessions are scheduled once a week. Frequency of sessions may change over time.
How long are the sessions?
All sessions [individual, couple and family] are 50 minutes in length.
What should I expect in the first session?
Therapy is about you - your emotions, your concerns and your perspective. It is my job to create an environment that can provide you with the space to feel safe, comfortable, supported, validated and affirmed. Initially, we will discuss any questions or hesitancies you have about [the process of] therapy. Afterward, we will begin to identify your goals for therapy that will be addressed together in future sessions.
Is my information confidential?
Communication between a client and therapist is completely confidential. No information is disclosed without prior written consent - this is done by signing a HIPPA form. However, there are some exceptions [required by law]:
-
If I suspect that an individual may be involved in child abuse or dependent adult/elder abuse.
-
If a client is threatening serious bodily harm to another person.
-
If a client intends to harm himself/herself/themself. I will make every effort to work with the individual to ensure their safety. However, if an individual does not cooperate, additional measures may need to be taken.
How much do you charge to supervise an MHC student?
Fees for field instruction are $125 per hour per student.
